KP provides Targeted Case management, also referred to as Coordination of Community Services (CCS). KP Staff assist clients in accessing Government services & community resources. KP’s efforts are driven by the individual’s defined goals of independence, integration and personal satisfaction.

Targeted Case Managers facilitate the community-based assessment and planning process by working with individuals, families, service providers, and other agencies and individuals to create an individualized plan based on a comprehensive assessment of needs for each person. In addition, case managers assist individuals and/or parents or guardians in navigating the service system.
